What this test measures

TSH (thyroid-stimulating hormone)
Shows whether the thyroid is underactive or overactive.
T4 (thyroxine)
The main hormone the thyroid gland releases.
T3 (triiodothyronine)
The more active thyroid hormone.
Anti-TPO antibodies
Antibodies against thyroid peroxidase, an enzyme in the thyroid. Raised levels suggest autoimmune thyroid disease such as Hashimoto's thyroiditis.

Who should take this test

  • Your TSH is raised and your doctor wants to know the cause
  • You have a family history of autoimmune thyroid disease
  • You are planning a pregnancy and your doctor wants to check thyroid antibodies

How to prepare

  1. No fasting needed.
  2. If you already take a thyroid tablet, give the sample before your dose unless your doctor says otherwise.
  3. If you take biotin (vitamin B7) or a hair and nail supplement, ask your doctor whether to pause it before the test. Biotin can distort thyroid results.

Common questions

What does a positive anti-TPO result mean?

It means your immune system is making antibodies against your thyroid. Many people with raised anti-TPO have normal thyroid function; your doctor will decide how often to recheck your TSH.

Do I need to repeat the anti-TPO test?

Usually not. Antibody levels are mainly useful for finding the cause, while TSH is what gets tracked over time.
